Take Control: Diabetes Group Program

Our Dietitian, Gauri, and Diabetes Educator, Amy, are offering a unique after-hours Diabetes Management Program running over three months.

This interactive and empowering program is designed for people living with Type 2 Diabetes who are ready to take control of their health. Join Gauri and Amy for a series of informative and engaging sessions that will help you tailor your diet and lifestyle to improve blood glucose control.

The program is delivered over four structured sessions, providing detailed, practical education and includes a Continuous Glucose Monitoring (CGM) trial.

Earlier this year, Gauri and Amy ran a similar program that was very well received. Participants reported feeling more confident and better equipped to manage their diabetes.

To join the after-hours group program, an initial daytime assessment with either Amy or Gauri is required.
Amy and Gauri are available on Fridays at Hornsby, with additional availability at other locations on other days.
